A payment is ready, and it waits. The approver is travelling, the mandate is unclear, the amount crosses a threshold nobody remembers setting. The chain moves by messages and by memory, and the payment goes out late, or it goes out because somebody was reassured.

A supplier account that changed. A payment to a country you have not paid before. An amount that is unusual for that entity. It sits in the same queue as the routine, and it is caught by whoever happens to look carefully.

The chain

It is checked against policy. The threshold, the mandate, the required approvers, and the limit on that entity.
The approval is gathered. From the right people, in the right order, with what they need in order to say yes.
The exception is separated from the routine. The payment that is unusual for this supplier, this entity, or this moment, and the one that is not.
The record is written. Who approved, on what basis, with which documents attached, and why that amount.
And it is reconciled. It left, it arrived, it matches, and the books agree.
And the routine continues. The next obligation is already forming.
